Telehealth informed consent
Last updated November 3, 2026. Please read this before you start your health intake. It follows Florida's telehealth law (section 456.47, Florida Statutes).
1. What telehealth is
Telehealth means getting health care from a licensed clinician without being in the same room.
With us, care happens through an online health questionnaire, secure messaging and, when needed, a video visit.
2. Benefits and limits
Telehealth can save you a trip and make it easier to stay in touch with your care team.
It has limits. Your clinician cannot do a physical exam. Your clinician may ask for a lab test, a video visit or an in-person visit. Your clinician may also decide that online care is not right for you.
3. Licensed clinicians
Your clinician is licensed to practice where you are located at the time of care.
4. Privacy and security
Your information is sent and kept using secure systems meant to protect your privacy.
As with any technology, there is a small risk of technical failure, such as a dropped connection or a delayed message.
5. Controlled substances
Controlled substances are not prescribed through this service.
6. Your right to say no
You may refuse or stop telehealth care at any time. Doing so will not affect your right to future care.
7. Emergencies
Telehealth is not for emergencies or urgent problems. If you have a medical emergency, call 911.
8. Your consent
By continuing to the health intake, you confirm that you have read this information and agree to receive care by telehealth.
